Step 2 of the Meydan Free Zone licence cancellation is a seven-question questionnaire that every applicant must complete before reaching the document upload. Nothing you write there changes the outcome or the AED 2,020 immigration card cancellation fee; it is an exit survey. But the fields are mandatory and the form will not move on with a blank, so it is worth knowing the questions in advance. Question 1: choosing the reason
The dropdown lists Meydan's standard reasons and an Other (Please Specify) option. Selecting Other adds a chip to the field and opens a second box titled "Other (Please Specify)" where you type the reason. "Discontinuation of business" is accurate for most closures and is what appears on the review page as two rows: N/A — Other (Please Specify) and Other (Please Specify) — Discontinuation of business. The reason is not a legal declaration and does not need to match the resolution wording, which uses "Voluntary winding up of Company" (see the resolution guide).
If the honest reason is cost, say so; Meydan's renewal package from AED 12,500 plus the annual audit is exactly why many single-purpose companies close once a project ends, and the survey is where Meydan hears it.
Questions 2 to 5: the rating questions
Question 2 is a multi-select of what attracted you. Questions 3, 4 and 5 all use the same four-point scale with no neutral option: Excellent, Very Good, Poor, Very Poor. Pick one on each; the portal does not allow a blank radio group. There is no scoring and no reviewer reads these before deciding on the file, so answer plainly.
One detail worth knowing: the options are the same on questions 3, 4 and 5 but the questions measure different things — expectations, service quality and support. Meydan reads the three together in its own reporting, so a Poor on support with Excellent on quality is a coherent answer, not a contradiction, and neither will be raised with you during review.
Questions 6 and 7: the free-text boxes
Both accept a one-word answer. "No" in each is common and is exactly how it prints on the review page ("Do you have any final feedback or suggestions for improvement? — No"). If you did have a recurring issue — for example delays on establishment-card cancellation that pushed you past your licence expiry — question 6 is the right place to record it, because the review page becomes part of your application record.
This appears under the step name in the left-hand progress bar, not next to the empty box, so people scroll up and down looking for the gap. Check that all four radio questions have a selection and both text boxes contain at least one character.
Answering as an agent or PRO on behalf of the shareholder
Many Meydan cancellations are filed by a PRO, a corporate-services agent or the accountant rather than by the shareholder. The questionnaire does not ask who is typing, and the review page attributes the answers to the company, so agree the reason and the ratings with the shareholder in a short message before you submit. Two practical points: the reason in question 1 should be one the shareholder would recognise if Meydan telephoned them, and question 6 should not be used to complain on the shareholder's behalf about matters they were not aware of. Keep the answers factual and short; the reviewer's attention goes to the resolution and the liquidation report in the next step, and a long narrative here simply lengthens the review page.
If the portal session times out mid-questionnaire, the answers already entered are kept when you use "Save & continue later", but the "Other (Please Specify)" text box is occasionally cleared on reload; re-check it before pressing Next. The chip showing Other (Please Specify) above the text box confirms the dropdown selection survived; the text underneath is what needs re-entering.

A one-person consultancy licensed in 2024 for a single client contract closed in September 2026 when the contract ended. Question 1: Other — "Discontinuation of business; client contract completed." Question 2: Price and Accessibility & Convenience. Questions 3–5: Very Good, Excellent, Excellent. Question 6: "Establishment card cancellation took nine working days." Question 7: "No." The application was approved after the 15-day notice period (how the notice works) with no query on the questionnaire; the only document Meydan looked at was the liquidation report.
